Reservations for Social Events and Seminars

To make a reservation for a Club Donatello function, please fill out the form below. If you prefer to mail a check, please do at least seven days before the event. The check should go to Club Donatello, 501 Post St., San Francisco, CA 94102. You may also make your reservations by fax: (415) 441-1816. If reserving by FAX or US MAIL, please include the information requested below.
